OYHC Home Care Ventilation/ Aerosol/ Oxygen therapy Services

Our goal is to provide quality healthcare services to clients in their home setting, thus minimizing their

dependence on institutional care.  Respiratory home care can be contributed to the following:

• Supporting and maintaining life

• Improving clients patients physical, emotional and social well  being

• Promoting patient and family self sufficiency

• Ensuring cost effective delivery of care.

The applicable categories of disorders include:

• COPD

• Cystic fibrosis

• Chronic neuromuscular disorders

• Chronic restrictive conditions

• Carcinoma of the lung

• Sleep Apnea patients (screening available)

Upon each visit the therapist will perform:

• Patient assessment (objective and subjective data, pre/post broncho-dilator studies, airway care and                 clearance methods)

• Equipment assessment

• Provide modifications in respiratory care plan based on patient concerns or identification of problems.

• Ventilation management

• Oxygen assessment and aerosol therapy.

These individuals will have improved quality of life, increased functional performance, and a reduction                             in the individual and societal costs associated with hospitalization. Visitation should be at least twice per

month.
